Taking

Black Heart damage in an active room will remove all Black Hearts from Isaac's health bar and convert them into demon familiars.

When the conversion happens, Isaac instead takes damage to the next available heart in the health bar.

If Isaac has only Black Hearts, taking damage will instantly kill him.

Any Black Hearts left on the ground will convert to demon familiars and fly away upon returning to the room.

Demon familiars will charge whenever they come into one of Isaac's cardinal directions. They charge in a straight line, dealing 1.75 × CurrentLevel damage per tick to enemies.

Demon familiars can also damage Isaac, and they will die if they hit him 2 times.

Upon clearing the room, all remaining demon familiars convert back to Black Hearts.

Since 1 demon familiar equals to 1 Black Heart, half a Black Heart can potentially be converted into a full Black Heart.

Whenever a Devil Deal spawns, there's a chance for an Angel Deal to spawn at the same time, similar to Duality, unless playing as Sarah that guarantees Angel Deals to spawn.

Angel rooms no longer contain items/pickups, and will only contain a different-looking angel statue.

Upon entering an Angel room, all of Isaac's Black Hearts will be donated to the angel statue in exchange for an Angel Room item, and one of the following if more exchanges were performed during the run:
• 1 exchange:

Gold Heart
• 2 exchanges:

Eternal Heart
• 3 exchanges: Eternal Chest; if playing as Sarah, it will permanently grant her flight
• 4 exchanges: Eternal Chest and

Eternal Heart

The cost for the exchange starts at 1 Black Heart, and is increased by +1 each time, up to a maximum of 5 Black Hearts. This progress is saved throughout the entire run.

For the purposes of the exchange, half a Black Heart is treated as a full Black Heart, so Isaac can, for example, exchange 0.5 Black Hearts and get the reward that requires 1 Black Heart.

Multiple exchanges can be performed by the same angel statue as long as Isaac has the necessary Black Hearts for it.

If Isaac does not have enough Black Hearts to make an exchange, they will remain as floating orbs around the statue until leaving the room.

Having only Black Hearts when performing the exchange will kill Isaac.

Touching the angel statue will fully heal up to 12 of Isaac's red heart containers.

Destroying the angel room statue will unleash all unused Black Heart orbs, and spawn 1-2 demon familiars in addition to the regular drops.
This item contributes towards the Seraphim transformation. Collecting 3 items from this set will grant Isaac flight and +3 Soul Hearts.
