Quantum Geode is a trinket in Fiend Folio, a mod for The Binding of Isaac.

Isaac fires tears in a triple spread, but once one of them hits an enemy, they all disappear.

If more than one of the tears hits an enemy at the same time, both will deal damage. This means that shooting enemies from up-close allows for triple damage.

Only affects tears. Other weapons, such as lasers, are not affected.

Geode Bonus: Tears disappear only once 2 of them land or hit an enemy.
