Star Sapphire is a trinket in Fiend Folio, a mod for The Binding of Isaac.

Spawns a familiar that wobbly orbits around Isaac and places itself in front of the nearest enemy or projectile.

The familiar blocks projectiles and deals 50% of Isaac's damage per tick to enemies it touches.
