Alpha and Omega is a passive item in Reverie, a mod for The Binding of Isaac.

This item's effect is based on the ending of the previous run you have ended with this item.

At the start of the next run, grants an item that is based on which ending is selected to end the current run.

Every ending's starting items and item effects:

Mom:
• Starting item: Mom's Ring.
• Item effect: x100 damage multiplier.

Mom's Heart:
• Starting item: The last item unlocked by defeating Mom's Heart.
• Item effect: Heals half red heart upon clearing a room.

Satan:
• Starting item: A random item from the Devil Room item pool.
• Item effect: +1 damage.

Isaac:
• Starting item: A random item from the Angel Room item pool.
• Item effect: +0.62 tears.

The Lamb:
• Starting item: A random item from the Devil Room item pool.
• Item effect: Spawns 3 black hearts upon entering a devil room.

???:
• Starting item: A random item from the Angel Room item pool.
• Item effect: Duplicates option items inside angel rooms.

Mega Satan:
• Starting item: Alpha and Omega.
• Item effect: On pickup, grants Key Piece 1, Key Piece 2 and a smelted Filigree Feather.

Hush:
• Starting item: Sacred Heart.
• Item effect: Permanently invincible.

Delirium:
• Starting item: A random item from a random item pool.
• Item effect: Spawns a random item from a random item pool at the start of every floor.

Ultra Greed:
• Starting item: A random item from the Shop item pool.
• Item effect: -50% pickup price.

Ultra Greedier:
• Starting item: A random item from the Shop item pool.
• Item effect: -67% pickup price.

Mother:
• Starting item: A random item from the Treasure Room item pool.
• Item effect: On pickup, grants 3 pairs of Knife Piece 1 and Knife Piece 2.

The Beast:
• Starting item: A random item from the Ultra Secret Room room item pool.
• Item effect: Spawns 3 Cracked Keys at the start of every floor.

Doremy:
• Starting item: A random item from the Planetarium item pool.
• Item effect: On pickup, spawns all items of a random Touhou Project character.
