Fourth Trumpet is a passive item in Anathema, a mod for The Binding of Isaac.

Darkens the game, the darkness effect gets stronger with each copy of this Buff.

Adds a small chance for every enemy to get struck by holy light beam every tick, the chance is bigger the more copies of this Buff Apollyon has. After successfully striking an enemy goes on a 3.3 seconds cooldown. The beam deals 0.5x damage + 5 flat damage.

This effect can only be triggered a set amount of times, that being equal to the amount of copies of this Buff (+4 if the current room is a Boss Room).

At 3+ copies:
When entering a hostile room, each enemy has a 50% (100% if it's a boss) chance to automatically be struck by the light beam.
