Codex Animarum is an active item in Eclipsed, a mod for The Binding of Isaac.

Spawns a Purgatory ghost that rapidly chases and deals contact damage to enemies equal to 50% of Isaac's damage per tick, 3.75 times per second.

Ghosts explode after 10 seconds, dealing 7 damage to nearby enemies up to 3-4 times, and destroying enemy projectiles. Isaac and the environment are immune to the explosion.

This item contributes towards the Bookworm transformation. Collecting 3 items from this set will make Isaac fire an extra tear 25% of the time.
