Hellfire is an unlockable passive item in Fiend Folio: RELOADED, a mod for The Binding of Isaac.

Enemies have a 13% chance to stay alive for 1.5 seconds more upon receiving lethal damage and shoot a Brimstone-like laser in the 4 cardinal directions.

The chance goes up to 33% with 10 luck.

The laser damages only enemies and lasts for 0.97 seconds. It deals damage equal to Isaac's, 15 times per second.

Enemies can still deal contact damage and fire projectiles during the additional alive time, or a total of 2.47 seconds added.

The lasers inherit Isaac's tear effects.

Also affects bosses and mini-bosses.

Defeat ???, The Lamb, Mega Satan, Mother, The Beast, Delirium, or Ultra Greedier in Fiend Folio as Tainted Azazel.
