Cain's Other Eye is an unlockable familiar in The Binding of Isaac.

Spawns a familiar that follows Isaac and copies all of his tear modifiers/stats, but with a 25% reduction in damage, unless playing as Lilith or Tainted Lilith.

The familiar fires randomly in the cardinal directions.
