Mephisto's Pact is a passive item in Lost and Forgotten, a mod for The Binding of Isaac.

Grants a Mephisto familiar that chases enemies and occasionally performs a dash attack that deals 12 damage.

Each floor Mephisto familiars show a different rule that Isaac has to follow.

If Isaac breaks the rule all Mephisto familiars become enraged and start chasing him dealing 3x damage. When one of them touches Isaac it deals damage and makes all of them disappear.

After entering a new floor without previously breaking the rule an additional familiar is granted.
