Heartache is an unlockable trinket in Fiend Folio, a mod for The Binding of Isaac.

Any damage taken will immediately grant a Broken Heart container that cannot be removed, rather than taking away from Isaac's health.

Isaac will die upon filling his entire health bar with Broken Hearts.

Taking damage like this does not affect the Devil/Angel Deal chance.

If playing as The Lost or Tainted Lost, the number of Broken Hearts granted when taking damage is multiplied by 3. They will die upon blocking 3 hits of lethal damage.
