Azazel's Rage is an unlockable passive item in The Binding of Isaac, added in the Repentance DLC.

Upon clearing a room, Isaac builds up rage that is indicated by the patch of skin spreading around his face, as well as 4 sections of the horn lighting up red one at a time.

After clearing 4 rooms, Isaac's head will flash red. Upon entering room with enemies, a large Brimstone laser is fired for ~3.33 seconds, that deals Isaac's damage per tick, 15 times per second.
