Twisted Pair is an unlockable familiar in The Binding of Isaac, added in the Repentance DLC.

Spawns 2 familiars that float by the left and right side of Isaac, and fire tears alongside him that copy his stats and tear effects.

Each familiar deals 37.5% of Isaac's damage. When playing as Lilith or Tainted Lilith, they instead deal 50% damage each.

This item contributes towards the Conjoined transformation. Collecting 3 items from this set will make Isaac shoot in 3 directions at the cost of -0.3 damage and -0.3 tears.
