Tainted Bethany is an unlockable character in The Binding of Isaac, added in the Repentance DLC.

Unable to use Red Hearts as health. She instead uses them as blood charges, as an alternative to charging active items. Any item that gives Red Heart containers will grant a Soul Heart instead.

Blood charges are displayed on the HUD and operate at the rate of 1 blood charge per half red heart. Bethany starts with 6 blood charges.

Rotten Hearts can also be used as charges - each gives 1 blood charge.

Blood charges can be used to use an active item without having enough regular charges to do so. This is displayed by the item bar having red instead of green charges. The amount of red charges is equal to how many blood charges are going to be consumed if the item is used.

Up to 99 blood charges can be held at a time.

Any stat ups granted by items are reduced by 25%.

Familiars and orbitals deal 25% less damage.

Familiars that drop pickups upon clearing a set amount of rooms will do so more infrequently (1-2 rooms increase).

Direct fire rate modifiers such as Mom's Perfume are not reduced.

The soft tears cap for Tainted Bethany is 4.43 instead of 5.
