Music Box is an active item in neverbirth, a mod for The Binding of Isaac.

On useGrants 20 seconds of damage immunity, turns tears red and plays music.

When the countdown ends, Isaac is forced to die.

An extra life is needed to survive this final death.

Using Music Box again does not restart or extend an existing countdown.

While heldIf a lethal damage event reaches the item while it has less than 12 charges, Music Box is consumed and automatically starts its protection once.

This item contributes towards the Seraphim transformation. Collecting 3 items from this set will grant Isaac flight and +3 Soul Hearts.

The automatic trigger requires no existing Music Box countdown or unresolved death debt.

The published implementation accepts an undercharged Music Box, including 1 to 11 normal charges; it is not restricted to exactly 0 charge.

The lethal-hit check compares incoming damage with current red hearts, soul/black hearts and twice the number of bone hearts, or accepts an already-mortal state.

It does not filter the damage event to enemy sources only.

Native shields or other damage cancellation may prevent the event from reaching this check.

The countdown is tracked for the individual character and is saved for continuing the run.

Changing rooms or losing the item does not cancel it.

A co-op teammate is not killed merely because another character's timer expires.

The protection cancels damage; it does not prevent the countdown's forced death.

Reusing the item cannot erase the death debt.

While protection is active, Plan C still consumes itself and damages vulnerable enemies, but its separate self-death is suppressed.

The Music Box countdown and its final death still remain.

Using Plan C while merely holding an inactive Music Box retains its delayed self-death.
