Mended knife is a familiar in Preyn's Collection, a mod for The Binding of Isaac. It is part of the Cursed Collection submod.

Spawns a knife familiar that launches itself in the direction Isaac shoots, dealing 25 damage up to 2 times and piercing enemies it hits.

Launching it at the red door at the end of Mausoleum/Gehenna II will open it. Unlike the knife pieces, the familiar will not be destroyed when doing so.

Part of the Cursed Collection mod.
