Keeper's Annoying Fly is an unlockable familiar in Repentance Plus, a mod for The Binding of Isaac.

Spawns a gray attack fly that orbits a moderate distance from Isaac and deals 1.5 damage per tick (22.5 per second), and absorbs both enemy projectiles and Isaac's tears.

For each Isaac's tear absorbed, its contact damage is increased by 3%.

The boost resets upon entering a new room.

Earn all Repentance Plus completion marks as Tainted Keeper.

This item contributes towards the Beelzebub transformation. Collecting 3 items from this set will grant Isaac flight, converts small enemy flies into Blue Flies, and permanently charms larger enemy flies.
