The Mark of Cain is an unlockable familiar in Repentance Plus, a mod for The Binding of Isaac.

Grants an Enoch familiar, which shoots Brain Worm tears that deal 3.5 damage, and shoots 1.67 times per second.

On death, if Isaac has any familiars besides Enoch, they are all removed and Isaac is revived with half a red heart in the same room.

After this, Enoch will become stronger:
• Tears will deal 25% more damage.
• Tears have a 7% chance to have the Ocular Rift effect.
• It will spew out 1-3 tears upwards at a short distance alongside its normal attack.

Defeat Satan as Tainted Cain.

This item contributes towards the Leviathan transformation. Collecting 3 items from this set will grant Isaac flight and +2 Black Hearts.
