Nightmare Grimoire is an unlockable active item in Sacred Dreams, a mod for The Binding of Isaac.

Spawns a book that falls on the ground dealing 5 damage and applying fear to nearby enemies.

The book then starts spawning a random set of friendly nightmare enemies.

Use BedTime Story as Azazel.

This item contributes towards the Bookworm transformation. Collecting 3 items from this set will make Isaac fire an extra tear 25% of the time.
