Ultra hard challenge
Ultra hard is an unlockable challenge in The Binding of Isaac, added in the Afterbirth+ DLC.

Starting items
Note: this list does not include characters' default starting items.
Additional notes
All enemies spawn as champions if possible.
Boss fights will be double trouble if possible.
Heart pickups are unable to spawn.
Curse filters
Based on the rules set in the .xml file, the following curses are always enabled or cannot be encountered:

Note: curses not listed here behave normally unless otherwise mentioned in the additional notes.
Completion reward
The following is unlocked for completing the Ultra hard challenge:
Samson starts with Child's Heart.
