Empty Cradle is a passive item in neverbirth, a mod for The Binding of Isaac.

While heldOnce per floor, the first qualifying hit records a heart category.

Clearing that same room grants a reward; another qualifying hit in that room before settlement downgrades it.
Red-heart category: a random full red, soul or black heart; downgraded: a random half red or half soul heart.
Soul-heart category: two independent resource rolls; downgraded: one roll.

Each roll gives 3 pennies, 1 key or 1 bomb.
Black-heart category: +1 damage for the floor; downgraded: +0.5 damage.

Empty Cradle can be found in the following item pools:

The following tags are applied to this item:

IV Bag damage, fake damage, Curse Room door damage, Devil deal payments, slot-machine damage, damage in Sacrifice Rooms and Blood Skull backlash do not count.

An explicit red-heart damage flag selects the red category.

Otherwise, having no soul hearts selects red; having any black hearts selects black; remaining cases select soul.

With mixed soul and black hearts, this can select black even when a soul heart is lost.

Leaving the room keeps the reward pending until that original room is cleared.

Hits in other rooms do not downgrade it.

A new floor resets eligibility, discards pending rewards and removes the damage bonus.
