Memory Disorder is a passive item in neverbirth, a mod for The Binding of Isaac.
Description
While heldStarting with the next room entry after pickup, changes Isaac into a random unlocked character whenever a room is entered.
Temporarily supplies the selected character's starting items and abilities while preserving the existing build.
Losing all copies restores the character identity held when Memory Disorder was acquired.
On pickupFor the rest of the run, eligible cleared endgame rooms guarantee a Void portal: Boss Rush, Hush, Mega Satan and the final Greed Mode boss room.
This remains after losing the item.

Notes
Room entries include revisiting rooms.
The identity changes halfway through a 30-game-frame darkening transition.
Extra copies do not create extra transitions.
On another identity change, the old temporary character components are removed and the new profile is supplied.
Permanent copies, ordinary acquisitions and the normal active/pocket inventory are tracked separately from borrowed components.
Losing the item removes borrowed components before restoring the acquisition-time identity.
Health is projected into the chosen character's supported health system.
Soul-only forms convert ordinary health into soul health; Keeper forms use their character health limits, and Lost forms use their native hidden health behavior.
Switching does not guarantee full healing.
Original identity, borrowed components, inventory and health tracking are saved for continuing.
Dead characters do not receive a pending identity change.
A paired-character switch is skipped if the required native partner preservation/removal support is unavailable.
The once-acquired Void guarantee applies to the run and the team, does not require continued ownership and does not duplicate an existing Void portal.
It only applies at the listed cleared encounters, not every boss room.
Unlocked vanilla profiles are candidates.
Modded characters require a separately registered compatible profile; installing another character mod alone does not automatically add it.
