False PHD is a passive item in The Binding of Isaac, added in the Repentance DLC.

On pickup +1 Black Heart.

While heldIdentifies all pills.

Converts all pills into their bad counterparts, but gives them positive secondary effects. Neutral pills don't gain a secondary effect.

Stat down pills will also grant +0.6 damage, or +1.2 if it was a horse pill.

Other bad pills will drop a Black Heart.

The item's effects work retroactively, giving stat boosts for each stat down pill taken before picking up the item.

When combined with PHD or other items that convert pills, the effects neutralize themselves allowing both types of pills to exist again.
