Mastema's Wrath item
Mastema's Wrath is an unlockable passive item in Mastema, a mod for The Binding of Isaac.
Description
When entering a hostile room, the enemy with the highest HP will be marked.
Killing that enemy grants +2 flat damage for the room.
If the enemy is a boss or mini-boss, the damage persists between rooms but will slowly fade away.
How to unlock?
Defeat The Lamb as 
Mastema.
