Variola is a passive item in Damnation, a mod for The Binding of Isaac.

On pickup Lose random amount of red hearts.

Gains a Horse Pretty Fly familiar.

While held Isaac becomes crown champion, offering random champion effects to all his familiars.
• Red: 2.6x HP and 3x collision damage.
• Yellow: 2x speed. Every 60s has a chance to spawn a Micro Battery or petrify all enemies.
• Green: Leaves a damaging creep. 1/7 chance to spawn a pill upon room clear.
• Orange: Head of the Keeper tears.
• Blue: Parasitoid tears.
• Dark Green: Explosivo tears. Explode on death (do not heart Isaac).
• White: 50% to prevent taking damage. Homing tears.
• Ghost: Spectral tears.
• Pink: Periodically release tears if close to enemies.
• Light Blue: Release tears in 8 direction on deah that inherit Isaac's damage and tear effects.
• Flash Grey: Repel Projectiles within a small radius.
• Flash Red: 35% chance to spawn random hearts upon room clear.
• Brown: Spawn random dip familiar upon room clear.

Familiars may gain some champion effects that's useless to them.

Smallpox was an infectious disease caused by the variola virus, which belongs to the genus Orthopoxvirus. The last naturally occurring case was diagnosed in October 1977, and the World Health Organization certified the global eradication of the disease in 1980, making smallpox the only human disease to have been eradicated.

This disease has a high death rate in the past. In Chinese history, most emperors are chosen because they've survived from Smallpox, so they won't suffer from the same disease again and could live for long. That's the reason why this item gives Isaac Crown Champion effect.
