Scapular is a passive item in The Binding of Isaac.

Once per room, when Isaac is down to his last half heart, he gains a soul heart.

Re-entering the room allows for the effect to be triggered again.

Cannot be triggered by self-sacrifice methods such as Blood Donation Machines or Devil Beggars.
