Description
Tainted Joseph is always in a nightmare state.

Nightmare Gapers will constantly come from off-screen in uncleared rooms and chase him during the nightmare state.

They deal Isaac's damage to nearby enemies after death

After one touches Tainted Joseph he gets 2 Broken Hearts, the nightmare state's intensity is reduced and all nightmare gapers on the screen die

Clearing a room increases the nightmare state's intensity and kills all nightmare gapers in the room, however entering a new floor reduces the intensity
