This item contributes towards the Celeste transformation. Collecting 2 items from this set will grant Isaac flight and constantly spawns little homing yellow tears around him dealing 20% of Isaac's damage.
This item contributes towards the Cyborg transformation. Collecting 3 items from this set will grant Isaac explosion and crush damage immunity, and spawn 8 bombs in a circle around him when using an active item with charges.
This item contributes towards the Cultist transformation. Collecting 3 items from this set will spawn a candle familiar which follows Isaac while regularly shooting 3 piercing blue fire projectiles at the closest enemy for 4-5 damage.
This item contributes towards the Conjoined transformation. Collecting 3 items from this set will make Isaac shoot in 3 directions at the cost of -0.3 damage and -0.3 tears.

This item contributes towards the Beelzebub transformation. Collecting 3 items from this set will grant Isaac flight, converts small enemy flies into Blue Flies, and permanently charms larger enemy flies.

This item contributes towards the Spun transformation. Collecting 3 items from this set will grant Isaac +2 damage, +0.15 speed and spawn a random pill.

This item contributes towards the Yes Mother? transformation. Collecting 3 items from this set will give Isaac a stationary knife that trails behind him and deals 2x contact damage.

This item contributes towards the Bookworm transformation. Collecting 3 items from this set will make Isaac fire an extra tear 25% of the time.

This item contributes towards the Oh Crap transformation. Collecting 3 items from this set will grant Isaac the ability to heal half a heart by destroying poop piles.

This item contributes towards the Fun Guy transformation. Collecting 3 items from this set will give Isaac +1 full red heart container.

This item contributes towards the Seraphim transformation. Collecting 3 items from this set will grant Isaac flight and +3 Soul Hearts.

This item contributes towards the Leviathan transformation. Collecting 3 items from this set will grant Isaac flight and +2 Black Hearts.

This item contributes towards the Guppy transformation. Collecting 3 items from this set will grant Isaac flight and spawn a Blue Fly 66% of the time when a tear hits an enemy.

This item contributes towards the Spider Baby transformation. Collecting 3 items from this set will spawn a spider familiar that applies random status effects when it comes in contact with enemies.

This item contributes towards the Bob transformation. Collecting 3 items from this set will grant Isaac poison immunity and make him leave green poisonous creep on the ground that deals 20 damage per second.
