While shooting, every 10-15 seconds Isaac will stop firing to release a burst of tears aimed upwards, that deal double damage.

The amount of tears in the burst is between 4 and 8, but can be increased based on Isaac's tears stat.

The effect is indicated by Isaac glowing red.

Using a pill creates a huge green puddle of creep in the direction that Isaac is facing, which rapidly deals 3% of Isaac's damage per tick to enemies.

The creep lasts for 4 seconds.
This item contributes towards the Spider Baby transformation. Collecting 3 items from this set will spawn a spider familiar that applies random status effects when it comes in contact with enemies.
