Can be acquired from the Mother chase scene in Mines/Ashpit II.

Spawns a knife blade familiar that follows Isaac.

When combined with the other piece, it forms a knife familiar that launches itself in the direction Isaac is shooting, dealing 25 damage to and piercing enemies it hits.

Can be launched at the red door at the end of Mausoleum/Gehenna to open the door leading to the Mother path, destroying the knife in the process.
