Costs 1¢ to play.

When given a coin, the beggar will lift one of the skulls up, revealing a coin, bomb, red heart, key, or Skatole. He will then shuffle the skulls around and wait for Isaac to bump into one of them, which will result in a payout.

The skull Isaac bumps into is not relevant.

Leaving the room before bumping into a skull will reset the prize.

Even though a Shell Game item pool exists, it is not used by this machine.

Possible payouts
• 2 of the previously shown pickup, or 3 if a coin was shown. These can be of any variant.
• Enemy Attack Fly.

Final payout
• Skatole

When destroyed
• Spawns several random pickups.
