Spawns a familiar that's attached to Isaac, and can be controlled in the direction Isaac is shooting. While Isaac is shooting, the familiar will shoot at the closest enemy.

The familiar inherits Isaac's damage, tears, range, shoot speed, tear effects, and tear modifiers.

Launching the familiar deals 3x Isaac's damage to enemies it hits.

Gello's tears deal 75% of Isaac's damage, or 100% if playing as Lilith or Tainted Lilith.
Unlock: Defeat Delirium as Tainted Lilith.
