Does not remove Soul Hearts or Black Hearts unless picked up as a character that cannot use red hearts.

Does not remove Bone Hearts unless picked up as The Forgotten.

Upon death respawns Isaac in the previous room and sets his red heart containers to 1 again.

The item is removed from Isaac's inventory after it respawns Isaac 9 times.

Can drop from the Super Pride mini-boss.
This item contributes towards the Guppy transformation. Collecting 3 items from this set will grant Isaac flight and spawn a Blue Fly 66% of the time when a tear hits an enemy.
Costs 2 Heart Containers when found in a Devil Deal, or 30¢ in an Angel Shop.
