After defeating a boss, a red crawlspace will appear in the middle of the Boss Room. Entering the crawlspace will bring Isaac to an extra boss battle of a higher difficulty.

Defeating this boss will grant a reward of two Ultra Secret Room items to choose from.

The crawlspace can only be entered once, but it can be entered at any time after it spawns.
Costs 2 Heart Containers when found in a Devil Deal, or 30¢ in an Angel Shop.
Unlock: Defeat Isaac as Tainted Jacob.
