Description
Red Heart containers are replaced with Rotten Heart containers. Any item that gives Red Heart containers will grant a Rotten Heart container instead, to a maximum of 6 containers.

Cannot acquire Red Hearts, Soul Hearts, Black Hearts, and Bone Hearts.

Has the ability to fly.

Shoots spectral tears.

Shooting a tear spawns a Blue Fly and a Blue Spider if the last ones are down. This effect has a luck-based chance to spawn a Locust or Arachnid instead; starting at 1 out of 80 at 0 luck and going up to 80 out of 80 at 79 luck.

Treasure Rooms and Shops are replaced with Rotten Rooms.

Spikes only damage Tainted Rotten for 1 Rotten Heart.

Devil Deal costs are reduced.

Blue Flies aggressively track enemies instead of rotating around Tainted Rotten.
