Above the item a curse icon will appear, and if the item is picked up that curse will activate.

Same curses cannot appear. If no curses left the item is free.

The item will be surrounded by spikes similar to A Pound of Flesh, and will disappear upon leaving the room.

Also appears on next run, but item quality will be 3 or more, and grants curse on entire run.
This item contributes towards the Leviathan transformation. Collecting 3 items from this set will grant Isaac flight and +2 Black Hearts.
