Generates a ladder that leads to a special Angel Room Shop at the start of each floor.

These shops will always contain the following:
• An item from the Angel Room item pool.
• Eternal Heart.
• Soul Heart.
• Key.
• Holy Card.
• Angel statue that can be fought for a Key Piece.
• Beggar or Rotten Beggar next to the entrance.
• Confessional, if unlocked.

Items and pickups can be on sale just like in normal shops.

The ladder will disappear once Isaac leaves the starting room.

The ladder will also appear in Greed Mode.
Unlock: Defeat Isaac as Jacob and Esau.
