Has a 90% to pay out with a prize, or 10% with nothing.

The prize has a 5% chance of being a random item from the Pain-O-Matic Machine item pool.

If this chance fails, an additional 75% chance is checked for dealing fake damage (similar to Dull Razor) which also has a 60% chance of gulping Isaac's trinkets, and a 25% chance for one of the following:
• 80% → an 80% chance for a Heart or Rotten Heart, or a 20% chance for another type of heart.
• 20% → +0.1 flat damage.

When bombed, spawns a random heart using the same rule as above.
Unlock: Defeat Mega Satan as Tarnished Samson.
