When Isaac gets hit, a slot machine appears over his head that rolls for a random effect.

The slots have a 30% chance to land on the same icon, resulting in a reward. An additional chance is given based on Isaac's luck stat, using the formula Luck > random(0, 10).

There are 6 possible rewards that can be given, each having an equal chance to be selected:

Blue Gem:
50% chance to spawn 2 random pennies; 50% chance for 1 random penny.

Cherry:
Red Heart that disappears after 2 seconds unless picked up.

Bell:
Shockwave that deals 25x Isaac's damage to nearby enemies, and pushes them away.

Horseshoe:
Equal chance to give +20% damage, +20% tears, or +3 luck for the current room.

Carrot:
Uses Free Lemonade, creating a large pool of damaging yellow creep.

777:
All of the previous rewards at once.

If damage is taken again while the slots are still rolling, another set will roll after the previous one finishes.
