Grants enemies a 33% chance to spawn a friendly red ghost upon death, which chases down enemies and deals contact damage to them.

Ghosts deal damage equal to 50% of Isaac's damage (2x Isaac's damage per second).

The ghost explodes after 7 seconds, dealing 7 damage to nearby enemies up to 3-4 times. This explosion cannot damage Isaac or the environment.

Enemies killed by ghosts won't spawn additional ghosts, except for those killed by the ghost's explosion.
