After being deployed, can be thrown in any direction using the shooting buttons. Will latch onto enemies, inflicting them with the Bruising effect while hooked, and then with Hemorrhaging when released.

The bruised effect makes enemies take +1 damage from all sources.

The Hemorrhahing effect makes enemies bleed, taking damage every 2 ticks, and creating puddles of damaging red creep, whilst spraying bloody projectiles that damage other enemies, dealing damage that scales with Isaac's.

Latching onto an enemy deals 50% of Isaac's damage. Letting go deals 100%.

The hook can be used to latch onto and grab pickups, even from behind obstacles. It can even grab item pedestals.

The hook can go through obstacles, but has a limited range, and will stop if it hits a wall.

The hook can be retracted earlier by using the active item button.

The hook can latch onto Randy The Snail and Pet Rock.

Can be used to steal bombs from Anti Golems.

Can unstick Sticky Nickels.
