Continuously firing tears for 2.35 seconds causes Isaac to fart (purely visual effect) and drop a Butt Bombs bomb.

Only one bomb can be dropped per firing cycle, requiring the player to let go of the firing buttons in order to start a new cycle. Taking damage will interrupt the cycle.
This item contributes towards the Oh Crap transformation. Collecting 3 items from this set will grant Isaac the ability to heal half a heart by destroying poop piles.
