• If in the starting room of a floor, and the room has not been left yet - +0.2 speed and increased fire rate for the rest of the floor.
• Once per floor, if every room except the Super Secret Room has been explored - spawns a Soul Heart and XXI - The World.
• If inside a Super Secret Room - first use spawns a Golden Cursed Penny; further uses activate The Poop and refund 2 charges.
• If inside a Shop - first use spawns 2 random Shop items for sale; further uses spawn 1-4 pennies.
• If inside a Treasure Room - first use spawns a random Glass Die; further uses will cause the D20 effect.
• If inside an Angel Room - spawns an Eternal Heart.
• If inside a Devil Room - spawns a Black Heart.
• If inside an Error Room - spawns Telepills.
• If inside a Clean Bedroom - first use spawns a random heart; further uses spawn a Cursed Penny.
• If inside a Dirty Bedroom - first use spawns a random heart; further uses spawn a Penny.
• If inside the Greed Mode Exit Room - first use spawns a penny; further uses produce a Butter Bean fart.
• If inside a Crawlspace - spawns a Sentry and refunds 2 charges.
• If inside a Black Market - first use grants an empty heart container; further uses spawn a Cursed Penny.
• If inside a Secret Room - first use spawns a Double Bomb; further uses spawn a Double Penny.
• If inside a Curse Room - first use spawns a random machine/beggar and a Troll Bomb; further uses will damage Isaac for half a heart.
• If inside an Arcade - spawns a random machine/beggar, and a penny.
• If inside a Vault - first use spawns 2 keys; second use spawns a Mimic Chest; further uses produce a Butter Bean fart and refund 2 charges.
• If inside a Challenge Room - if not completed, grants a one-time +0.5 damage bonus; second time produces a Butter Bean fart; or if the room is completed, spawns a random chest.
• If inside a Library - spawns a random trinket.
• If inside a Sacrifice Room - spawns a Blood Sack or Trash Bag.
• If inside a Dice Room - first use will spawn every type of Glass Dice; further uses produce a Butter Bean fart and refund 2 charges.
• If inside a Planetarium - first use activates Soul of Isaac; further uses spawn 2-4 random Book of Virtues wisps.
• If inside an Ultra Secret Room - first use spawns 3 copies of Cracked Key; further uses produce a Butter Bean fart and refund 2 charges.
• If inside the Downpour Entrance - spawns a key.
• If inside the Mines Entrance - spawns a Double Bomb.
• If inside the Mausoleum Entrance - spawns 2 Blended Hearts.
• If inside any other Alt-Path Entrance - spawns Cracked Key, Heart (half), and a Troll Bomb.
• If a White Fireplace is in the room - spawns 4 arrows pointing to it.
• If in a Mirror Room - if Isaac does not have the Lost's Curse effect, applies it; otherwise breaks the mirror; or if neither criteria are valid, produces a Butter Bean fart and refunds 2 charges.
• If in the Mines chase entrance and the button is pressed - spawns Knife Piece 2.
• If fighting a boss inside a Boss Room, Mini-Boss Room or Boss Rush - temporary tears up.
• If in a combat room with active enemies - fires 3 tears to random enemies that deal double damage.
• If in a normal room with no active enemies - spawns a Penny and refunds a charge.
• If in Home - first use spawns a random Dad item; second use spawns Cracked Key; further uses spawn a random pickup.
• If in the Mega Satan fight - first use spawns Tick and displays the "I'M WITH YOU!" fortune text; further uses display "EVERY STEP OF THE WAY".
• If in the Beast fight - first use recharges the Holy Mantle shield, further uses have the default pickup spawn chances.
• If a Golden Boom Fly is inside the room - transforms it into a Mern.
• If a Mother Orb is in the room - broken, does nothing.
